New Customers

We welcome you to Elite Bodyworkers.  Due to the nature of our business, we do require all new customers to complete a New Customer Medical & Massage Intake Form. You can:

• Fill this form online by Clicking Here *RECOMMENDED **; or
• Print out and bring an advanced copy with you; or
• Fill one out in the office. IF YOU CHOSE THIS METHOD, PLEASE PLAN ON ARRIVING AT LEAST 15 MINUTES AHEAD OF SCHEDULED APPOINTMENT TIME.

For In-Clinic Massage Services

Notice of cancellation is needed 24 hours prior to appointment to avoid 100% payment.  Additional service fees include the following and will be added to your order accordingly:

1. Before 9am or after 9pm in clinic: + $10
2. On major holidays in clinic + $20

For Club EB Membership Program

The authority to debit the account will remain in affect for a MINIMUM of your selected program membership.  All change or cancellation requests must be applied for in writing via e-mail or mail.  If you choose to cancel your membership it must be canceled before the 25th of your last month otherwise your membership will be automatically renewed.

• The membership fees are NOT refundable and NOT transferable.
• All fees are paid in advance and due in full upon sign up.
• All payments/fees will be debited without notice/receipt from Elite Bodyworkers.
• Elite Bodyworkers reserves the right to assign this authorization without notice.
• The membership credits are valid for 3 months and then are automatically released from the account.
• I further understand that this is a Term Contract that will Automatically convert to an OPEN-END Contract without any notice to you.
